No silent work: every PHX Good action enters Operations

This is a company operating rule. Directions, research, decisions, assignments, implementation, tests, releases, reviews, deliveries, blockers, failed attempts and material outcomes must enter Operations. A routine review that finds no change receives an explicit no-change record. Missing evidence is recorded as unknown, not completion.

Each record carries the actual date, owner, current state, privacy-safe scope, source or visible unknown, limitation and next action. Proposed, in progress, blocked, delivered, reviewed, accepted, published and verified are different states. Recording work does not prove a provider action, completed referral, person helped, outcome or another person's acknowledgment.

Reuben follows this rule for PHX Good work. His personal-adviser role and work outside PHX Good remain outside the company record. Private, privileged, sensitive client, personal, credential and financial details stay out; where details cannot be public, we keep the smallest accurate privacy-safe status and explain the limit.
